To transfer your primary care to another provider at Family Care:
- Before your next appointment, decide if you would like to transfer your care to Sarada Schossow, PA-C or Brooke Barkley, NP.
- If you have no preference, we’ll just set you up with the provider that best meets your availability.
- Your new provider will review your records and history to be prepared to continue the your current plan of care at your next visit.
- Everything else about your healthcare will stay the same!
If you would like to transfer to another facility, we have two ways to complete a records release form:
- Online: Please complete and sign this online form to either send or receive records.
- You do not need to print or scan anything to use this option.
- Print / Sign / Scan / Return: To complete a release on paper, please download the appropriate linked form below:
- Send Records TO Family Care. Another provider’s records going to Family Care.
- Release Records FROM Family Care. Family Care’s records going to another provider.
- When you are done, you may upload the form to our office using our File Upload page.
