Starting January 1, 2026, patients who have the NC State Health Plan will receive discounted primary care services by selecting a specific primary care provider through their insurance plan.
- Patients will need to log in to their eBenefits portal or through their employer’s HR link and select “Update Primary Care Provider.”
- They next need to search for your practice name and click “select as your PCP.”
- Last step, they must click the green SAVE button.
Once you have selected Dr. Sabrina Mentock, Dr. Elaina Lee, Sarada Schossow, PA-C, or Brooke Barkley, FNP-C, you will receive a confirmation, see their name on your new insurance card images, and start receiving lower costs at Family Care!
Patients who complete this simple process will receive a lower co-pay when they have a visit with the practice they selected. The lower co-pay is:
- Standard & 70/30 PPO Plans: $15 (Non-preferred= $40/$50)
- Plus PPO Plan: $10 (Non-preferred= $30/$40)
This action helps patients reduce their out-of-pocket costs. We encourage you to proactively reach out to your insurance and make sure you are saving money each visit by selecting any Family Care provider as your Selected PCP.
